BRIDGING MEMBERSHIP – GoodLife Corporate Short-Term Membership Application Form
Dear Members,
If you would like to join the UNIFOR & IAMAW GoodLife Corporate membership program prior to the next annual sign-up date of April 1, 2015, you may pick up an application form for a short-term membership from Lidia Kafieh (UNIFOR Local 2002 members) at Terminal 1, Ground Level/Bunker Health and Safety Office or Linda Mastromattei (IAMAW Local 2323 members) at C/O IAMAW Shop Committee Office, Ground Level Terminal 1 or Shop Committee Office Bay 8 or at the Local Lodge Office at 2580 Drew Road.
Start dates are available on the 1st and 15th of the month. Applications must be received a minimum of 2 weeks prior to your requested start date. The cost is $39.00 plus tax per month and must be paid in full lump sum payment by Visa or Mastercard only. Please submit your completed form in person, for processing to the appropriate offices indicated above. This offer is only available to active UNIFOR Local 2002 and active IAMAW Local 2323 Members and their family members living in the same household.
Please note that if you are an existing GoodLife member, currently paying your membership fees through a GoodLife club, you will also need to request and submit a completed Personal Membership Cancellation Form to Lidia or Linda, when submitting your short-term membership application.
