Employee Assistance Program –Claim Forms

Dear brothers and sisters,

Please be advised that all EAP counselling expenses incurred since July 14, 2011, are to be submitted to Sun Life using the Extended Health Care Claim Form only. No additional EAP claim form is required. EAP counselling claims can be sent to any of the addresses provided on the Sun Life claim form.

NOTE:  Members have 90 days after the transition date to submit all their EAP counselling expenses incurred before July 14, 2011. These expenses must be sent to ClaimSecure using the two-form process previously used at Air Canada.
